For female, gender-expansive and sexual minority persons, basic anatomy itself is an oft overlooked and incompletely characterized keystone component of this complex ecosystem.<\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Maintaining sexual health and the capacity for sexual activity after the biological, physiological, and psychosocial sequelae of cancer therapy can be fundamental to quality of life for cancer survivors.<\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Specifically, the function of and damage to erectile tissues (including the bulboclitoris and nipple-areolar complex, which are primary organs responsible for sexual pleasure) in female and gender-diverse persons with cancer remains an under-represented scientific domain &#8212; so much so that the anatomic basis of orgasm has been largely omitted in cancer literature, toxicity measurement, and medical coding; rather, difficulty with orgasm after cancer treatment is attributed to psychogenic etiologies in female persons, despite the substantial damage of cancer treatments to the erectile tissues and associated nerves and vessels. In addition, endocrine transitions, particularly the menopausal transition (whether natural or treatment\u2011induced), introduce rapid shifts in estrogen, progesterone, and androgen signaling that alter genital and breast tissue integrity, vascular responsiveness, and sensory function. These hormonal changes can profoundly shape sexual function and may heighten susceptibility of erectile and pelvic tissues to injury from radiation and other cancer therapies, yet these mechanisms are poorly understood and seldom incorporated into trial endpoints or survivorship care.\u00a0 Similarly, damage to prostate sensation and capacity for anal-receptive intercourse and the role in sexual pleasure and sexual health in certain sexual-minority males have also been overlooked.<\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">In contrast, maintaining capacity for pleasurable sexual activity after cancer treatment via the study of male erectile tissue function has received sustained scientific, structural and financial investment, enabling accelerated scientific advancements, development of effective precision interventions, and access to advanced technologies to prevent and mitigate these toxicities.<\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\"><em>We have an opportunity to address these systemic and structural inequities. <\/em><\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">A more equitable framework for scientific and biomedical understanding of sexual health and capacity for sexual activity must move beyond penile-vaginal intercourse and reproductive capacity, and instead incorporate rather than omit the functional and physiologic anatomic basis for sexual pleasure, endocrine transitions, and therapy-related tissue injury for female, gender-diverse and sexual minority persons with cancer.<\/p>\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\"><em>Shifting the paradigm of sexual health for female, gender-diverse, and sexual minorities persons with cancer will<\/em>:<\/p>\n<ul class=\"n8H08c UVNKR\">\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Better align of cancer care with sexual rights that protect all people&#8217;s rights to fulfill and express their sexuality and enjoy sexual health, with due regard for the rights of others and within a framework of protection against discrimination (<a class=\"XqQF9c rXJpyf\" href=\"https:\/\/www.google.com\/url?q=https%3A%2F%2Fwww.who.int%2Fteams%2Fsexual-and-reproductive-health-and-research%2Fkey-areas-of-work%2Fsexual-health%2Fdefining-sexual-health&amp;sa=D&amp;sntz=1&amp;usg=AOvVaw3ixK9qix8WdgGsBa_-QVKR\" target=\"_blank\" rel=\"noopener\">WHO<\/a>).<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Normalize and affirm sexual pleasure as an important aspect of sexual health in cancer survivorship for women and gender\/sexual-minority persons.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Empower clinicians with evidence for incorporating the anatomy, physiology, endocrinology, and pathophysiology of organ function related to sexual pleasure in a more equitable way during clinical interactions and in providing tailored clinical decision-making.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Correct the gendered inequities in investment of scientific and biomedical resources for research and development in maintaining organ function for sexual pleasure to accelerate scientific advancement.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Provide a more equitable and just distribution of advanced technologies that may preserve sexual function after cancer treatment.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Ethically empower respect for persons and justice in research and clinical care through equitable inclusion of women and sexual\/gender minorities.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Address health disparities for women and sexual\/gender minorities in illness burden, treatment experience, and sexual health outcomes.<\/p>\n<\/li>\n<li class=\"TYR86d wXCUfe zfr3Q\">\n<p class=\"CDt4Ke zfr3Q\" dir=\"ltr\">Enable communities to translate new evidence into action through advocacy for equity and access.<\/p>\n<\/li>\n<\/ul>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][\/et_pb_section][et_pb_section fb_built=&#8221;1&#8243;
